Request a Primary Care Appointment Online
If you’re a current Baystate Health patient, you can now schedule an appointment online. To schedule an annual physical, follow-up, or same-day sick visit, select “Request Appointment” with one of our providers. You can schedule appointments online or through the MyBaystate portal.
Please note: Not all providers offer online scheduling. “Request Appointment” will only appear next to providers who are accepting online appointments.
Our Services
If you have a general health concern, we are ready to see you and help you address it. Our team includes doctors, nurse practitioners, and other healthcare professionals—many of whom have been here and lived in the community for many years. If you’re 18 or older, you can benefit from our expert, compassionate primary care, which includes:
- Chronic disease management, such as for diabetes, high blood pressure (hypertension), and high cholesterol
- Disease prevention
- Education about health conditions
- Follow-up visits
- Health maintenance and preventive care
- Mammograms
- Regular physicals, including Medicare wellness visits
- Point-of-care testing for several routine tests, such as urinalysis, hemoglobin testing, A1C testing, etc.
- Sick and urgent visits
- Telehealth, phone and patient portal visits
- Vaccines for the flu and other illnesses
Our providers consistently receive high ratings for patient satisfaction and quality of care. We have a diverse team of providers who are fluent in several languages in addition to English, including:

We have convenient parking, as well as a covered entrance for the comfort of patients being dropped off at the door. Our office includes separate hallways and exam room doors for patients and providers for added privacy and safety.
Additional Services
In addition to our primary care services, you have access to lab services from Baystate Reference Laboratories (BRL), located within our building for your convenience. We are located within Baystate Health & Wellness Center – Longmeadow, which includes providers from a wide range of specialties if you need additional care.
We work closely with our colleagues at nearby Baystate Medical Center in Springfield, as well as providers throughout the Baystate Health system.
What Is Internal Medicine?
Internal medicine doctors and nurse practitioners provide patient-centered care for all the medical concerns and conditions an adult may experience. This is different from other medical specialties, which typically provide care to patients of any age (family medicine), a specific type of patient (women, children, etc.), patients with particular conditions or providing a particular type of care.
Internal medicine often serves as the first step in a patient’s healthcare journey. Our providers care for all adults. You must be at least 18 to see one of our internal medicine providers.
You’ll have peace of mind knowing a single internal medicine practice is familiar with and caring for all your medical needs.
Specialty Care
We are part of an integrated clinical network that includes specialists, hospitals, home care, laboratory and imaging services. If your condition requires that you see a specialist, we can coordinate your care with specialists within our high-quality network. All our providers use the same medical records to make your care seamless.
