Your Partners In Working With Baystate Health's Providers
The Baystate Health physician liaison team works closely with community providers in western Massachusetts. Our job is to make it easier for you to find the right specialists across our regional hospitals and ambulatory practices and navigate the referral process.
We’re here to assist you with:
- Referring your patients to our providers for specialty testing and care
- Accessing our electronic medical record (EMR) for patients you have referred to us
- Connecting with specialists for clinical guidance
- Providing access to our PCP Clinical Advice Line
- Taking part in our educational programs

Our Available Services For Community Providers
We know how busy our community providers are. Choose from the following options to find out how our team can work with you:
The Physician Liaison team can help you navigate the referral process and provide options for referring patients for specialty care. Email us to get started with your referral.
You can also download referral forms for pediatric and adult patients and learn more about our providers and specialty practices on our Refer a Patient page, or find the right specialist for your patient.
If you have a login ID and password and want to view the information of a patient you have referred to us, log in to our EMR.
If you do not have a login ID and password, email us to request access to our EMR. We can speed your request if you include these items in your email:
- Your name
- Your title
- Preferred contact information
- Your affiliated practice
From 8 am to 5 pm Monday through Friday, you can reach out to our specialists for clinical questions and advice if you have questions about a patient’s symptoms, treatment plan, or other aspects of their care.
The specialists on call for the Primary Care Provider Direct Clinical Advice Line will respond to you within the same business day. If you have questions or would like to learn more about this valuable resource, email the Physician Liaison team.
The Physician Liaison team coordinates Lunch and Learn meetings with community providers who are interested in learning more about diseases, treatment options, clinical guidelines, and more from our specialty physicians.
We also facilitate Meet and Greet sessions when we have a new provider join Baystate Health. This helps us let community providers know of new specialists and new specialty care available for their patients.
