BMI-Associated Labor Induction (BALI): A Prospective Trial

The purpose of this research study is to understand if there are any benefits to the subject and their baby by being delivered at 39 weeks compared to waiting until labor or the subject is 41 weeks and 3 days pregnant. We are specifically interested if earlier delivery will help lessen the chance of cesarean section. In this study, subjects will have a scheduled induction date in the 39th week (subject's obstetrician or midwife would give subject medications to start labor).

Eligibility:

Inclusion Criteria:
Age ≥18 years, gestational age between 38 weeks 0 days and 38 weeks 6 days, nulliparous (no prior pregnancy delivered past 20 weeks), pre-pregnancy or first trimester BMI ≥ 35 kg/m2, plans to deliver at Baystate Medical Center.

Exclusion Criteria:
Plans for an induction prior to 41 weeks, major illness with increased risk of adverse pregnancy outcomes (pregestational diabetes with or without medication, gestational diabetes on medication, hypertension, cardiac disease, renal insufficiency, autoimmune disorder), pre-eclampsia, gestational hypertension, fetus with major/lethal anomaly or aneuploidy.

Time Commitment:
9 weeks. Subjects will be scheduled for induction anywhere between 39 0/7 weeks and 39 6/7 weeks and be followed for 6-8 weeks postpartum. Subjects will receive a phone call between 6-8 weeks postpartum. The subject's baby will be followed until they receive the follow-up phone call 6-8 weeks postpartum.
