At Baystate Health the Health Information Management (HIM) departments fulfill all requestions for medical information. Requests for records should be made on the Authorizartion for Release of Information form. Download the form by clicking on one of the following links:

- Once you complete the Authorization for Release of Information form, you may mail, fax or personally deliver it to one of our HIM departments.
- Upon release of this valid authorization, HIM staff will be happy to provide you with the requested records.
Record Release Fees
Baystate Health does not charge a fee for medical records sent to a health care provider at the request of the patient for the purpose of continuing care.
Any other copies requested by the patient or by attorneys, insurance companies, subpoenas or Workers Compensation will be charged at the rate of $0.63 for each page 1 – 100 and $0.33 for each page 101 and greater. Payments may be made in the form of cash or check; credit cards are not accepted.
